A valid Ontario health card is needed for OHIP-insured services. Availability may change as the practices fill.

What should I bring?
Bring your Ontario health card, a medication list and any relevant medical information. Reception will tell you if anything else is needed.
Can my family join too?
Yes, ask reception about registering each family member when you call. Each person needs their own first appointment.
Can I book my first visit online?
Please call for your first visit. Online booking is for existing Riverwood Family Practice patients only.
Is this a walk-in clinic?
No. Riverwood Family Practice is an appointment-based family practice. Please book before arriving.
What about my previous medical records?
Reception can explain how to request relevant records from your previous clinic. That clinic may charge a transfer fee.
What if I need accessibility or language support?
Tell reception when booking so we can discuss the support available.
